To improve the overall quality, efficiency, and value of the healthcare services provided by the nation’s health centers, and to celebrate their recent achievements in providing care to nearly 30 million patients, the Health Resources and Services Administration (HRSA) provides Quality Improvement Awards. HRSA’s quality improvement awards recognize the highest performing health centers nationwide as well as those health centers that have made significant quality improvements from the previous year.
The U.S. Department of Health and Human Service’s (HHS’) announced more than $117 million in quality improvement awards to 1,318 health centers across all U.S. states, territories and the District of Columbia. HRSA-funded health centers will use these funds to further strengthen quality improvement activities and expand quality primary health care service delivery.
Crescent Community Health Center is delighted be the recipient of the Health Center Quality Leader, Clinical Quality Improvers, Advancing Health Information Technology for Quality, and Patient Centered Medical Home Recognition Awards resulting in more than $77,000 in funds given by HRSA and HHS’.
As a Health Center Quality Leader, Crescent achieved the best overall clinical performance among all health centers, placing in the top 30% of the adjusted quartile rankings for clinical quality measures. The Clinical Quality Improvers Award recognizes that Crescent demonstrated at least a 15% improvement on a clinical quality measure from 2018 – 2019. The Advancing Health Information Technology (HIT) for Quality Award acknowledges Crescent for optimized HIT services for advancing telehealth, patient engagement, interoperability and collection of social determinants of health to increase access to care and advance quality of care between 2018 and 2019.
Lastly, the Patient Centered Medical Home Recognition recognizes Crescent’s patient centered medical home (PCMH). The PCMH model ensures care delivery coordinated through a primary care provider which ensures necessary care when and where the patient needs it and in a manner they can understand.
