UnityPoint Health Finley Hospital is excited to establish a Cardiac and Pulmonary Phase III program. With Finley’s focus on growing heart and lung care services for the tri-state region, this is yet another way we can ensure our community members get the care they need, when they need it.
There are currently two phases to Finley’s Cardiac and Pulmonary Rehab program. Phase I sometimes includes a hospital stay, but is focused on evaluation, education and a determination of next steps after a cardiac event. Phase II begins as an early outpatient in Finley’s Cardiac and Pulmonary Rehab program. During Phase II, Finley’s Cardiac and Pulmonary Rehab team offers supervised exercise time and classes to closely monitor patients once they begin rehabbing after a cardiac or pulmonary event.
The launch of the Phase III program is considered an extended outpatient program. Members of the community who have experienced a cardiopulmonary event can continue with monitored exercising right here in Finley’s new state-of-the-art cardiac facilities once they’ve graduated from Phase II. Patients will also continue to have wellness and health coaching as part of Phase III.
UnityPoint Health Finley Hospital is recruiting new members for a Patient and Family Advisory Council (PFAC). The council is intended to provide real-time feedback from Finley patients and their families in order to enhance the patient experience at Finley Hospital. Finley patients and/or family members of patients (both current or former) are invited to join Finley’s PFAC.
Experience shows that best outcomes are reached when health organizations involve patients and their families in the health care process and decision making. The PFAC takes that concept a step further by inviting patients and families to be involved at a planning and program development level for hospital practices.
The collaborative group meets 8-10 times per year, during the months of February-June, as well as August-November. Meetings are held the third Tuesday of the month, lasting approximately one hour. Interested members are asked to complete an online application form found at unitypoint.org/pfac.
